Riverview School

Riverview School’s Post High School Transition Program for young adults, ages 17–21, is called GROW—Getting Ready for the Outside World. The curriculum includes direct instruction in practical academic subjects but also provides students who have completed high school with the social, vocational, and independent living skills they need to function as independently as possible. Additionally, Riverview's transition planning services are second to none and help students prepare for life after graduation. Structured much like a college campus, GROW students live in dorms, take classes and participate in an extensive internship program. They learn how to self-advocate, make mature choices and take responsibility for all aspects of their lives, including selecting and planning their own social activities. Additionally, students gain firsthand experience in a variety of work settings, helping them to develop good work habits and increased awareness of personal strengths and interests. There are 4 flagship programs that make Riverview’s GROW Transition Program the gold standard in post-secondary education for students with complex special needs: Café Riverview, Second-View Thrift Store, Project SEARCH, and our ability to provide support while students take mainstream college classes at Cape Cod Community College.

RISE (Reaching Independence through Supported Employment)

Shrewsbury Public School’s RISE (Reaching Independence through Supported Employment) program is a post-secondary program designed for students with disabilities, ages 18 – 22, who qualify for additional learning opportunities following their high school experience. The goal of RISE is to provide students with the skills and experiences they need to successfully and independently navigate their local community. The focus is to increase students’ overall independence with functional academic life skills, employment opportunities, and community access. Students receive direct instruction in each of these skill areas that includes hands on practice opportunities. Skills are taught and practiced within the classroom environment than worked on in the community in real-life situations. The RISE program looks to provide students with meaningful employment opportunities in their local community. Students build upon their previous work experience and explore various career areas. They practice career preparation skills and functional employment skills within the classroom, then learn to apply these skills into an employment setting. Our faculty, clinicians and staff are deeply committed to providing excellent care to the diverse populations served by Boston Medical Center, VA Boston Healthcare System, and our community health center partners. We embed trauma-informed care into our clinical practice and we provide care that is responsive and tailored to the needs of the many patients who receive care at our institution. Center for Living & Working, Inc. (CLW) was established in 1975 in Worcester, Massachusetts. CLW was established by a core group of young adults with disabilities dedicated to the empowerment of all persons with disabilities to live as independently as possible. The primary mission of CLW has not changed since its inception. However, programs and services have constantly evolved and improved to meet the changing needs of consumers with disabilities. CLW is a consumer directed, multi-faceted agency in Central Massachusetts offering a variety of services for people regardless of disability type and age. CLW’s consumers live in diverse areas within the region from rural to inner-city neighborhoods and suburban towns. They represent numerous ethnic and cultural populations. At least 51% of CLW’s staff and Board of Director’s are individuals with a range of physical and cognitive disabilities, including persons who use wheelchairs, have mobility issues, have emotional or neurological disabilities, or are deaf or hard of hearing. CLW embraces the independent living philosophy which states that all individuals with disabilities should have the same civil rights, options, and control over choices in their own lives as do persons without disabilities. Our staff is representative of the consumers we work with. CLW serves over 4,200 individuals annually with a budget of $4.5 million dollars.

Founded in 1979, the Polus Center has 40 years' experience designing and implementing innovative projects that support people with disabilities and members of other vulnerable groups. In Massachusetts the focus of the Polus Center is on employment and training initiatives for adults and youth who have disabilities. In collaboration with the Carroll Center for the Blind, the Polus Center recently began implementation of the new Employment Now Initiative sponsored by the MA Commission for the Blind, which offers internships and job placement in Boston area hospitals, and also continues a similar project called Envision Work sponsored by the Partners for Sight and Boston Foundations. Throughout Massachusetts the Polus Center sponsors an innovative after-school and summer program for legally blind youth age 14-22 called the Envision Success Project, in which students tour businesses and workplaces and do internships in areas of their potential career interest. Polus has long been a partner of the MassHire Workforce system, having supported the implementation of the Disabilities Employment Initiative V Career Pathways project, and recently being awarded a grant from the Commonwealth Corporation to work with the MassHire North Central Career Center and MRC to provide a CVS Pharmacy Tech training program for young adults with disabilities. Polus provides workplace training for people with disabilities and for people who support their employment through non-profit agencies and municipalities. In addition to employment and training initiatives in Massachusetts, for the past 20 years the Polus Center has worked around the world supporting landmine survivors and other victims of war and conflict, including an art therapy and rehabilitation program for Syrian refugee war-wounded children in Jordan and support for coffee farmers in Latin America and the Democratic Republic of Congo.

Transitions to Work provides a 12 week intensive skill training and internship program, placement services, and ongoing support for participants and employers. Transitions to Work engages employers as corporate partners to consider adults with disabilities as qualified, committed candidates for appropriate employment opportunities and to raise awareness about inclusive hiring. Transitions to Work graduates earn jobs that provide earnings and a sense of purpose. Eligible participants for the program are between the ages of 18 and 35, have a documented disability, and are excited to find a job. Transitions to Work is a collaboration among the Ruderman Family Foundation, Combined Jewish Philanthropies, and Jewish Vocational Service. We create a Positive Experience Design (PED) to help individuals transition from school to an adult life in the community. The group works with our team to create a Positive Experience Design (PED) customized for the individuals to help transition from school to an adult life in the community. YWU believes that a meaningful life includes a home, a job, family, friends and social opportunities with peers - able and otherwise. These opportunities include becoming a non-participating member of a college athletic team, sitting on the bench at games and hanging around campus with the team; moreover, these opportunities range from aforementioned sports groups, to 1:1 peer mentoring, to any other sort of group an individual feels they want to join and grow with.
